Eu, abaixo assinado(a), declaro estar ciente de que a participação em atividades físicas no WinnerCircle24FitClub envolve riscos à saúde, como lesões físicas, mal-estar, complicações de saúde e, em casos extremos, morte.

Declaro também que:

1. Estou em plenas condições físicas e mentais para participar das atividades propostas.

2. Assumo total responsabilidade sobre minha saúde e integridade física durante as atividades.

3. Comprometo-me a informar à equipe organizadora qualquer condição pré-existente que possa afetar minha participação.

4. Isento o WinnerCircle24FitClub, seus organizadores, instrutores, parceiros e patrocinadores de qualquer responsabilidade civil ou criminal por acidentes, lesões ou danos que possam ocorrer durante ou após minha participação nas atividades.

AUTORIZAÇÃO DE USO DE IMAGEM

Autorizo o uso da minha imagem (fotos, vídeos ou gravações) feita durante as atividades do FitClub para fins promocionais, publicitários e institucionais, sem que isso implique em qualquer compensação financeira ou solicitação prévia.        

WAIVER AND RELEASE

I, the undersigned, understand that participation in any fitness-related activity at WinnerCircle24FitClub involves a risk of injury, illness, or even death. I voluntarily choose to participate in such activities, including but not limited to group workouts, fitness challenges, and nutrition discussions.

I acknowledge that:

1. I am physically and mentally able to participate in these activities.

2. I will notify the instructor of any conditions that may affect my ability to participate.

3. I assume full responsibility for any injuries or damages that may occur to me as a result of participation.

4. I release and discharge WinnerCircle24FitClub, its instructors, affiliates, and sponsors from any and all liability, claims, demands, or causes of action.

MEDIA RELEASE

I grant permission to WinnerCircle24FitClub to use photographs, videos, or recordings taken during FitClub sessions for promotional and marketing purposes.

I have read and understand this waiver and agree to its terms.
